Direct Client/Family Support

The focus of this service is to provide a detailed plan of support care alternatives that will assist families in making an informed decision of their senior loved ones available options. 

At ICC we want to provide options that balance the client’s desire for independence with the need for support when diminished abilities interfere with normal daily activities being completed independently.  Achieving this delicate balance ensures: dignity of the individual, quality of life and safety in the home plus peace of mind for family members and caregivers. 

ICC provides an individualized assessment that focuses on the resident and their current environment as well as incorporating the role various family and informal caregivers currently play.  This is followed up with a detailed report outlining the various options to be discussed with regard to benefits and inherent obstacles and challenges surrounding each option.
